Winter Care for Glowing Skin!


Your Face Needs Extra Care in Winter!

Just like your car needs snow tires or chains and your outdoor faucets need insulated covers, you need to take special care to winterize your skin too. During this long cold spell, you can see how winter weather can dry-out your skin. Extremely low temperatures with little or no humidity coupled with harsh winds can deplete the skin of its natural lipids and your skin can get dried, flaky and a bit irritated (this is called "winter itch"). To keep feeling moist and dewy even during our week of real winter, follow three important tips to see what a difference they can make.

Achieve a Healthy Winter Glow!

In the winter, moisture is essential for soft skin.  Apply moisturizer when the skin is damp, right after a shower or hand washing. During the dry, cold days, consider using an oil-based moisturizer.  Moisturizers with a higher concentration of oil can guard more effectively against a moisture loss. 

Lukewarm Water.
When it's bone- chilling cold, nothing feels better than a steaming hot bath or shower, but please keep in mind that hot water robs skin of its natural moisture and can cause your skin to get very dry. It is best to shower or bathe in lukewarm water. If you can't bear this rule -- I personally love a long hot shower -- try to keep your warm showers short and try showering only once per day. This also means skipping the hot tubs in winter (another rule even I must struggle with).

Hands, Feet & Lips.
Hands, feet and lips can suffer terribly in winter, so make it a practice to baby them. Put on moisturizer and gloves before you head outdoors.  Moisturize your lips with Vaseline. A simple lip balm may not offer enough protection.  You can lather up your feet in thick moisturizer and sleep in cotton socks at night. Grab those gloves and keep them on! Protecting hands from the cold air and low humidity plays an important role in preventing dry, flaky skin. Make sure the linings of your gloves are very soft and made from material that does not irritate your skin.

Dear Friends, Colleagues and Patients:

Greetings!  I hope you stayed warm and safe during this unusually long cold spell. During these last two weeks, I think most of us have stayed home to avoid the icy roads.  Despite the inconvenience of the cold weather and
snow, the plateau has never looked more stunning. From its snow covered lawns brimming with shimmering lights to the fragile icicles hanging from the street lamps, the wintry beauty is a joy to experience.  

Despite the beauty it brings, the cold weather can play havoc with your skin.  To keep your face fresh in the winter, apply moisturizer to your face before going out into the cold and cover your face with a scarf to protect it from the harsh wind. Please see below where I am including a few other skin care steps that you can take to ensure that you do not get rough, dry or irritated skin.

It is important to let you know that I will be on assignment and teaching in Mexico, so I will be out of the office from Tuesday, January 6th  and not returning to the office until Wednesday January 14th. I have a few openings between now and the time that I leave, so if you call now, I will do my best to fit you into my schedule.

This month, I am focusing on a little known condition of the skin called Melasma which can cause large, unsightly brown spots.  Typically this condition can be found in higher numbers among women who have darker complexions and who are from the Mediterranean region, Middle East, Asia or South America.  
My patient, Lisa R., is very unusual because she is blonde, blue- eyed and light skinned. We successfully treated Lisa's Melasma with a new procedure called Melanage. For Lisa's complete story, please scroll below.

This month we are happy to include a wonderful nutrition tip from Sammamish-based nutritionist Deborah Enos.  I have also included my wine tip for the month and my recipe for roast Stuffed Georgian Chicken, which is wonderful and savory meal to include during your holiday week. Aside from being so delicious, it is so easy to make. You can just pop it in the oven and enjoy the feast!

Lisa R--My Experience with Melanage

I'm 43 years old, fair-skinned, blue-eyed and blond.  Whenever I was in the sun, I always got freckles, but I never really tanned.  About seven years ago, during the summer, while I was visiting a friend in Idaho, I had been out in the sun and when I returned indoors, I suddenly noticed I had brown spots on my forehead. The spots did not fade or go away. This had never happened before and I found it troubling, so I went to a dermatologist who said the brown spots were the result of my having a hormonal imbalance.  

The brown spots grew larger and more noticeable on my forehead while a new patch emerged and traveled up around the side of my mouth and down to my chin.  Then a new spot showed-up on my cheek. There were large brown spots above my eyes. One spot ran down the side of my eye and below my eyebrow. The spot above my right eye was as large as a silver dollar and there was a slender one-half inch spot under my eye. I went to three different dermatologists who provided treatment, but the spots would not go away. One of the dermatologists gave me a hydroquinone cream (2% solution)  which I tried and there was a very slight fading, but nothing significant.  

There was a name for my condition; it is called Melasma.  Usually it can afflict women who have darker complexions and whose ethnic origins are from South America, the Middle East or Asia.  It is unusual for Melasma to afflict a very light complexion, but in my case, the outbreak of Melasma was linked to my hormones.  I started reading everything I could about Melasma. Soon I was trying
everything I could do to get rid of these brown patches that could not even be concealed with make-up.  I tried every over-the-counter lightener cream I could find. Some were very expensive, but nothing worked.  I sought naturopaths and aestheticians.  I tried IPL therapy and had a total of five treatments.  Some of the pigment darkened and flaked off, but there was no real significant change made to the brown spots.

I started seeing a naturopath to get an update on supplements.  I also wanted to find out everything I could about Melasma.  There wasn't anything she could find out, but she recommended a really
great doctor who was a facial plastic surgeon-Henri P. Gaboriau M.D. She said, 'If you go to Dr. Gaboriau, he will give you the straight scoop on exactly what can be done or not done. '

Initially, I made an appointment with Dr. Gaboriau to have my moles removed.  He explained exactly what Melasma was-that it was really deep pigmentation.  He also explained that the IPL therapy did not work because it only treats the top layer of the skin and does not penetrate the deeper layers.  He recommended a treatment called Melanage that has been formulated specifically for Melasma. At this point, after all I had been through, Melanage was really my only hope.  

Dr. Gaboriau and his staff were very thorough in explaining what I would have to do to enable the treatment to be successful.  I had a pre-op appointment that explained every step.  First a vitamin
liquid, then a goopy mask, had to stay on my face for five hours.  Every day I had to follow a very specific skin care regimen that consisted of cleanser, calming balm & sunscreen, vitamin A pads to-be-used for 7 days, and a Melanage hydroquinone cream that was customized for my skin type.  I was required to do this skin care routine for 7 days.

On the second day my skin looked sunburned. The 3rd day my skin started to peel in layers and it was still red.  The next few days there was a succession of peeling. I continued to use the skin care regimen for seven days and every day my skin showed improvement.  By the seventh day, the bleaching agents had gotten down into the deep layers.  My skin had also tightened and I could see that the brown spots were spots were faded.  Several weeks later I could hardly believe the difference.  People who know me and have seen me without makeup, cannot believe the difference--It  is 90% + better than what it was.  

I feel like my old self again! For years I didn't want to have my picture taken because I could only see those brown spots. Now when I look in the mirror, I say 'okay, there I am.'   This is the best thing I could have ever done. Overall, my skin looks new.  I almost cried.  It's nice to have a doctor I can trust.  ---LR

Nutrition Tip

RE: Drink a hot beverage to help resist the temptation of holiday goodies!       
According to certified nutritionist Deborah Enos,
"When faced with a delicious holiday buffet, most people have a difficult time saying no to many of the great looking goodies. The mistake we make is to go to a party feeling really hungry.  When you feel those hunger pangs, control goes out the window.  But if you sip on a cup of hot tea or any other hot beverage (no, not a hot toddy or a calorie- rich mocha!) it can help to reduce your hunger, giving you more self control as you face the buffet table laden with scrumptious goodies."


 Henri's Stuffed Georgian Chicken

This meal is so tempting and satisfying on these cold winter nights!

30 min prep| 90 min roast | SERVES 4 to 6

2 whole 4-5-pound uncooked chickens

8 tablespoons butter
2 onions, peeled and finely chopped
3 cloves garlic, finely chopped
1 cup basmati rice
3/4 cup dried cranberries
½ chopped pistachios
Salt and freshly ground black pepper
One small sprig chopped rosemary (a little goes a long way!)


1. Remove any fat from cavities of chickens, and place fat in a wide saucepan. Add 4 tablespoons of the butter and place over medium heat. When butter melts, add onions and garlic. Sauté until onion softens and begins to brown, about 5 minutes.

2. Discard any remaining hard bits of chicken fat. Add rice and chopped cranberries, and pistachios to pan. Stir well, and add 2 cups water and salt to taste. Bring to a boil, cover and reduce heat to as low as possible. Cook for 15 minutes. Meanwhile, heat oven to 425 degrees.

3. Season rice mixture with salt and pepper to taste and stir in rosemary. Spoon rice into cavities of both chickens.

4. Place chickens in a roasting pan and baste with remaining butter. Place in oven and roast until skin is crisp and golden and juices run clear when pierced near the thigh with a knife, 1 1/2 to 2 hours. Allow chicken to rest before carving. Serve each portion of chicken with some stuffing.

The Roast Georgian Chicken is a deluxe feast which is why I always recommend making two roasts instead of only one.  The first chicken is thoroughly devoured and the second roast is only there as a polite afterthought, just in case you or your guest would like a little bit more. The meal is so inclusive that you might only want to add a simple salad of buttery bibb lettuce dusted with olives, parmesan cheese, croutons, and  drizzled with a touch of truffle oil and maybe some very light dinner biscuits, or a perfectly simple pull-apart round loaf of buns.  Bon Appetit!!

Originally from France, Dr. Henri P. Gaboriau obtained his first medical degree at the School of Medicine in Paris. He then transferred to Tulane University School of Medicine in New Orleans, La., where he received his second medical degree in 1994. He stayed at Tulane Medical Center to do his residency in the field of Otolaryngology/Head and Neck Surgery.
